Registered Nurse- Home Based Primary Care
Duties include, but not limited to the following:
1. Demonstrates grade-appropriate and consistent critical thinking skills. 2. Provides a full range of nursing care to patients with complex care needs utilizing the nursing process based on the American Nurses Association Standards of Nursing Practice. 3. Demonstrates consistent clinical competence in providing nursing care. 4. Uses equipment correctly and accurately interprets data. 5. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit. 6. Initiates and implements an interdisciplinary plan of care based an assessments. 7. Performs ongoing assessment of patient condition and response to treatment. 8. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. 9. Assesses the patient's learning needs, learning preference, and readiness to learn. Modifies teaching approach based on knowledge of human behavior, motivations, and reactions to situations. 10. Accurately documents in the medical record patient finding, assessments, and care provided. 11. Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes. Work Schedule:

A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded Grade Determinations:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Moderate lifting, 15-44 pounds; Light carrying, under 15 pounds; Pushing 1 hour; Walking 2-4 hours, Standing 2-4 hours; Kneeling 1 hour; Repeated bending 1 hour; Climbing, legs only 1 hour; Both legs required; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid permitted); Working closely with others; This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, kneeling, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or during patient procedures and care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Use of fingers, both hands, and near and far vision is required.
